(OLYMPIA, WA) -- The following are recent disciplinary actions taken, including suspension or revocation of licenses, certifications, or registrations, against Washington State health care providers by Washington State’s Department of Health Systems Quality Assurance Office.
The office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions such as medical doctors, nurses and counselors.
In March 2010 the Nursing Assistant and Surgical Technologist Programs charged certified nursing assistant Sabdaniayu Tarigan (NC10077772, ST00003280) with unprofessional conduct. She allegedly treated a patient after her credentials expired.
In February 2010 the Nursing Assistant Program denied the application of Kristi D. Tompkins (NA60083993). She left her shift without notifying a patient, or her employer. The patient woke with extreme abdominal pain, and she wasn’t there to assist the patient.
In March 2010 the Massage Program charged Sharon L. Ballenger (MA00010904) with unprofessional conduct. She allegedly tested positive for controlled substances on more than one occasion.
In March 2010 the Nursing Assistant Program charged certified nursing assistant Loyd Hanna (NC10092436) with unprofessional conduct. His license to practice as a health care assistant was indefinitely suspended due to failure to comply with a previous order.
In March 2010 the Board of Pharmacy charged pharmacy assistant Lonnie L. Hunt (VB00047003) with unprofessional conduct. She allegedly diverted controlled substances from the place where she worked. She was convicted of soliciting possession of a controlled substance.
In March 2010 the Hypnotherapist Program charged Maurice D. Brooks, Sr. (HP10001487) with unprofessional conduct. He was convicted of social security representative payee fraud, and felony theft of public money.
In March 2010 the Denturist Program terminated the order of probation against Roy J. Clark (DN00000255).
In March 2010 the Licensed Social Worker Counselor Program terminated the order of probation against Jean C. Cook (LW00005818).
In March 2010 the Nursing Assistant Program charged certified nursing assistant Rita A. Hill-Wonder (NC10052799) with unprofessional conduct. She allegedly pinched a patient’s nose and clapped her hands loudly in front of a patient’s face.
In March 2010 the Nursing Assistant Program released Jasmina Kahrimanovic (NA60120358) from terms and conditions set against her registration.
In March 2010 the Dental Commission entered into an agreed order with dentist James R. McAnally (DE00007153). He practiced below the standard of care when providing treatment to patients. He can’t practice orthodontics for five years.
In March 2010 the Nursing Assistant Program released certified nursing assistant Brian A. Rucci (NC60092049) from terms and conditions set against his certification.
In March 2010 the Nursing Assistant Program charged certified nursing assistant Rex E. Tan (NC60034911, NA60022001) with unprofessional conduct. He allegedly attempted to steal, or borrow a total of $9,500 from two patients.
In March 2010 the Board of Pharmacy terminated the order of probation against pharmacy assistant Alvin M. Uhl (VB00072550).
In March 2010 the Registered Counselor Program charged Karen A. Young (RC00047750) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her registration.
In March 2010 the Nursing Assistant Program vacated a prior order, granted the reactivation application of Sharley R. Shore (NA00031810), and placed her registration on probation for three years. She was convicted of welfare fraud and two counts of false verification of welfare form. She must comply with terms and conditions set against her registration.
Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.

The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700.
Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
