 (OLYMPIA, WA) -- The following are recent disciplinary actions taken, including suspension or revocation of licenses, certifications, or registrations, against Washington State health care providers by Washington State’s Department of Health Systems Quality Assurance Office.
The office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions such as medical doctors, nurses and counselors.
Snohomish County In December 2009 the Board of Pharmacy charged pharmacy technician Shirley G. Molina (VA00056467) with unprofessional conduct. She allegedly took large quantities of hydrocodone from her place of employment, and was convicted of acquiring a controlled substance by deception and subterfuge.
In December 2009 the Nursing Assistant Program charged certified nursing assistant Cameron E. Sheppard (NC10049044) with unprofessional conduct. He was convicted of assault.
In December 2009 the Nursing Assistant Program charged certified nursing assistant Michael R. Tucker (NC10081370) with unprofessional conduct. He allegedly made inappropriate comments to multiple patients.
King County
In December 2009 the Medical Commission placed the license of physician Peter Balousek (MD00035044) on probation for three years. The care he provided to a patient was below the standard and created an unreasonable risk of harm to the patient, resulting in patient death.
In December 2009 the Nursing Assistant Program granted the application of certified nursing assistant Courtney Renae Barber (NC60102052) and placed her certification on probation for 18 months. She entered into a deferred prosecution for assault — domestic violence. She must comply with terms and conditions set against her certification.
In November 2009 the Nursing Commission denied the application of registered nurse Sharon Renee Beadle (RN60100143). She applied to reactivate a registered nurse license but has never held a license to practice as a registered nurse in Washington
In December 2009 the Dental Commission placed the license of dentist Richard Calvo (DE00009044) on probation for three years. He was convicted of trafficking in stolen property, and theft. He must comply with terms and conditions set against his license.
In December 2009 the Medical Commission released physician Matthew R. Crouthamel (MD00047066) from terms and conditions set against his license. Dr. Crouthamel’s license is unrestricted.
In December 2009 the Nursing Assistant Program charged certified nursing assistant Debra E. Marx (NC10012040) with unprofessional conduct. She allegedly administered intravenous medication to a patient about six times. This is outside of the scope of practice for a nursing assistant in Washington.
In December 2009 the Dental Commission charged dentist Paul S. Rotwein (DE00007883) with unprofessional conduct. He allegedly provided dental services below the standard of care while placing a bridge on a patient, performing a root canal on a different patient, and placing a crown on another patient.
In November 2009 the Health Professions and Facilities Program charged Seattle Motor Inn (FS60013366) with violations of WAC 246-360. The inn allegedly had widespread maintenance and sanitation violations.
In September 2009 the Medical Commission charged physician Virginia T. Stevens (MD00030208) with unprofessional conduct. She allegedly practiced below the standard of care while performing a breast reconstruction and using a medical device for an off-label purpose. She also allegedly abused alcohol and had sex with a patient. She signed an agreement not to practice medicine until she receives permission from the commission.
Pierce County
In November 2009 the Registered Counselor Program denied the application of Perry T. Dyer (RC60042907). He was convicted of assault — domestic violence, and failed to appear at the pre-hearing conference.
In December 2009 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Uriah T. Hill (CO60113797) and placed his certification on probation for two years. He was convicted of possession of methamphetamine, and theft. He must comply with terms and conditions set against his certification.
In December 2009 the Health Care Assistant Program charged Kimberley J. Templeton (HC00147463) with unprofessional conduct. She was convicted of seven counts of violation of the uniform controlled substances act.
In December 2009 the Massage Therapy Program charged Aisha N. Viera (MA60022044) with unprofessional conduct. She was convicted of child assault.
Island County
In December 2009 the Nursing Assistant Program charged Krysten N. Kimmel (NA60002313) with unprofessional conduct. She was convicted of assault.
Whatcom County
In November 2009 the Health Professions and Facilities Program charged the Shangri-La Motel (FS00002320) with violations of WAC 246-360. The motel allegedly had widespread maintenance and sanitation violation issues.

The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700.
Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
