 (OLYMPIA, WA) -- The following are recent disciplinary actions taken, including suspension or revocation of licenses, certifications, or registrations, against Washington State health care providers by Washington State’s Department of Health Systems Quality Assurance Office.
The office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions such as medical doctors, nurses and counselors.
Snohomish County
No reported cases in June or July.
King County
In July 2010 the Registered Counselor Program terminated the order of probation against Kevin B. Baker.
In June 2010 the Nursing Commission reinstated the license of licensed practical nurse Monica Denice Blansett, also known as Monica D. Hopper and placed her license on probation. She must comply with terms and conditions set against her license.
In July 2010 the Dental Commission charged dentist Ronald Brun with unprofessional conduct. The care he provided allegedly fell below the standard when he created an inappropriate treatment plan for a patient, and failed to keep adequate documents of the patient’s condition.
In July 2010 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Kennith Wayne Fowler and placed his certification on probation until April 2012. In 2009 his credential to practice as a registered counselor was placed on probation. He must comply with terms and conditions set against his certification.
In June 2010 the Board of Pharmacy placed the license of pharmacy technician Sara M. Gagliardo on probation for five years. She was arrested for unlawful possession of a controlled substance — heroin, and admitted using illegally obtained narcotics. She must comply with terms and conditions set against her license.
In July 2010 the Medical Commission charged physician Ki Hyun Oh with unprofessional conduct. He allegedly failed to evaluate a patient’s condition further, and failed to follow the advice of radiologic consultants resulting in a misdiagnosis.
Island County
No reported cases in July
Pierce County
In July 2010 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Theresa Michelle Stach and placed her certification on probation until August 2012.
In August 2009 her credential to practice as a registered counselor was placed on probation. She must comply with terms and conditions set against her certification.
Skagit County
In July 2010 the Secretary of Health placed the license of physician Wirt A. Hines, III on probation for seven years. He was convicted of criminal attempt of sexual assault on a child. He must comply with the terms, conditions, and restrictions set against his license
Whatcom County
In July 2010 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Raymond J. U. Hite and placed his certification on probation.
In February 2010 conditions were placed against his counselor registration. He must comply with terms and conditions set against his certification.
Island County
No reported cases in July.
Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.

The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700.
Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
